In July 2019, Mysterio returned to WWE programming, making a surprise appearance at the Extreme Rules pay-per-view, helping Kevin Owens win a steel cage match against Dolph Ziggler. The following night on Raw, Mysterio defeated Ziggler to become the new United States Champion, thus becoming a Grand Slam Champion in the process.
On October 6 at Hell in a Cell, Mysterio lost the United States championship back to Ziggler in a triple threat also involving Owens. At Crown Jewel on October 31, Rey Mysterio was part of Team Hulk Hogan (which also consisted of Ali, Shorty Gable and Rusev) as they defeated Team Flair (consisting of King Corbin, Sheamus, McIntyre and Ziggler) in a five-on-five tag team match.
Mysterio then challenged Andrade for the United States Championship at Survivor Series on November 24 but failed to win the title.
